Review boundaries

Staging uses fake `.test` accounts, deterministic scanning by default, and disabled production actions. The dashboard may show demo controls, but product email sending, Stripe, real AI providers, Gmail API access, mailbox reads, file upload, and production data are intentionally disabled.
